The login for MySQL on a server running Plesk is the following:
user: admin
password: the plesk admin password. You can find out how to get that at the following:

This will work on most installs and is very straight forward.
SSH into the server that you installed Plesk on.
Find the home directory of Plesk (generally /etc/psa) and cat the “.psa.shadow” file.
This will display the admin password.
